Description - Provide comprehensive care management and ongoing support to older adults living in the community - Conduct regular in-home visits and assessments throughout your assigned territory - Advocate for clients and help them navigate healthcare, housing, financial, and social service systems - Assist clients in accessing benefits, entitlements, and community resources - Develop individualized service plans and monitor client progress - Collaborate with family members, healthcare providers, and other members of the client's support network | ||||||||||||||||||||

Skills - LMSW or LCSW - Experience working with older adults, care management, case management, or related human services fields - Knowledge of Medicare, Medicaid, Social Security - Excellent interpersonal, communication, and advocacy skills - Strong organizational, documentation, and time-management abilities - Ability to work independently while remaining an active member of a collaborative team - Proficiency with Microsoft Office and electronic documentation systems | ||||||||||||||||||||
