Traveling within the BronxGeneral ResponsibilitiesJASA operates an Assertive Community Treatment Team (ACT Team) that serves Bronx older adults ages 55 and older who are diagnosed with a Serious Mental Illness (SMI) and have demonstrated a need for intensive services, due to a history of instability in the community.
The Family Specialist, Bronx Assertive Community Treatment (ACT) Older Adult Services, is responsible for family needs assessment and implementation of family intervention strategies, using CBT and other techniques, including strength-based therapy and motivational interviewing to work with families of ACT Team clients; has lead responsibility for integrating family goals and services with the tasks of all team members.
Primary Responsibilities
In conjunction with other ACT Team staff members, provide evidence-based mental health interventions to individuals who are enrolled in ACT Team servicesConduct home and/or institutional visits Administer mental health screening toolsComplete the Cultural AssessmentAdminister the Family Needs Assessment Coordinate/Facilitate Family meetings Provide resources and support to clients and family members to prevent caregiver burnoutDevelop, implement, monitor, and document treatment plansComplete required case records, reports, and statistics within mandated timeframes, utilizing electronic systems (EHR, funders’ systems, Google Docs and Sheets, Microsoft Excel, etc.)Maintain cooperative relationships with public and voluntary agencies, programs, services, and facilitiesParticipate actively in regular individual and group clinical supervision, including interdisciplinary team conferencesParticipate in staff meetings, program planning, trainings, conferencesRepresent the agency at events and meetings as neededEnsure all services meet regulatory requirementsRotate 24/7 on-call duties with other team membersPerform other duties as assigned
QUALIFICATIONS:
Master degree in Social Work, Mental Health Counseling or similar requiredPossession of a New York State professional license; LCSW, LMSW, LMHC, LCAT, LMFT, Clinical Psychology, or similar- preferredAt least two years working in a, mental health setting or with an older adult population or similar - preferredExperience with community visits or intensive treatment work- Preferred Familiarity with family needs assessmentsKnowledgeable of resources for older adults and their familiesExperience with using Family Intervention Strategies Training/Experience in Psychoeducation and other Family Support ServicesDesire and ability to work collaboratively and engage in quality improvement activities as part of a committed, integrated teamExcellent interpersonal skills and the ability to communicate effectively and concisely both in writing and orallyStrong attention to detail with the ability to solve problems; to elicit, comprehend, and synthesize accurate and specific information; prioritize work; to work independently; and to excel in a high-performance cultureAn understanding and appreciation of the roles of race, religion, ethnicity, sexual preference, and individual values as they relate to serving clientsProfessional maturity and strong commitment to JASA’s mission Basic computer skills; familiarity with an electronic client record system preferred
