Columba Services Inc. is a nonprofit organization dedicated to helping individuals who have struggled with homelessness. Many of our residents have experienced significant trauma in their lives and are struggling with mental illness and/or substance use disorder. We provide a safe and stable environment for all the individuals we serve while treating them with Respect and Dignity.   A Harm Reduction approach is used at Columba Services.

Essential Functions:
Reasonable accommodations may be made to enable individuals with disabilities to perform essential functions.
•        Provide trauma-informed, case management services to individuals who are struggling with substance use disorder (SUD)and/or mental illness (MI).
•        Create person-centered goals for service planning with individuals.
•        Assist individuals with housing stability, budgeting, health and wellness.
•        Work with individuals to break through barriers in attaining goals assist with advocating for themselves and successfully moving toward self-sufficiency.
•        Connect individuals to resources for medical, mental health, substance use, vocational, and other resources.
•        Participate in the intake and admission process for individuals.
•        Co-facilitate groups around harm reduction, daily living skills, health relationships, budgeting and others.
•        Immediately reports incidents or sensitive situations to supervisors.
•        Update community referral sources for individuals.
•        Maintain and expand relationships with community providers.
•        Actively participate in staff and community meetings.
•        Perform other related duties as assigned.
