RESPONSIBILITIES:Provide services ranging from concrete services (benefits and entitlements), to case assistance and case management, group programming, advocacy, resources and referrals
Conduct intakes, screenings and needs assessments as assigned
Facilitate support groups as assigned by supervisor
Assist participants with case management and assistance needs
Provide information and make referrals/provide linkages to other appropriate services
Maintain, and complete timely case records for all participants in "STARS" Peerplace database including progress notes, case contacts and all case summaries, and produce monthly reports
Comply with all funder service standards and contract requirements
Participate in case presentations, staff meetings, field visits and regular training
Submit work schedules and statistics in a timely manner as required
Keep supervisor abreast on an ongoing basis and in supervisory meetings of outstanding work priorities
Participate in community-building activities sponsored by program and agency and encourage participants to support these activities
Willingness to work a flexible schedule, including some weekend and/or evening hours as necessary
Be available to work on either Thanksgiving Day or Christmas Day, to be decided in consultation with the Director, as well as other holidays and special events as necessary
Be aware of programmatic targets and goals and work towards these in all activities provided to participants
Engage in professional development and apply learned knowledge towards using evidence-based best practices
Engage in data analysis processes conducted by the Evaluation and Quality team to determine strengths and areas for improvement and follow action steps in the Continuous Quality Improvement (CQI) process
Maintain cooperative, supportive relationships with Hartley House and Hudson Guild and programs/employees
Perform other tasks as assigned
