| Description This role will perform full physical therapy evaluation (collecting learner history, using standardized functional tests and outcome measures, determining PT diagnosis/prognosis, constructing learner-centered plans of care).
 Ability to create home exercise programs along with learner education
 Follow Individualized Family Service Plan (IFSP) and develop methodologies for reaching each child’s specific developmental goals
 Provide therapy services to improve gross motor development, address large motor concerns, and increase strength and muscle development
 Provide therapy to assist with achievement of gross motor milestones at appropriate levels of development and ability
 Ensure that progress is documented, and caregivers are active participants in intervention through addressing specific goals, coaching sessions, and communication with the entire team

 Prepare progress reports
 New Jersey Physical Therapy (PT) license
 Maintains license status in accordance with state requirements
 Early Intervention Experience OR Equivalent Experience
 Pediatric Experience

| Notes Home Based Service Delivery Experience
 Strong Clinical & Interpersonal Skills
 Must have at least one year of experience working with children on the autism spectrum.
 Excellent written and oral communication skills
 Excellent computer skills
 Ability to travel – services are provided in the client home
 Valid CPR License
 Valid NJ Driver’s License
