Position Summary

The Social Worker Team Leader provides leadership, clinical supervision, and direct services within the Case Management Program (CMP), supporting frail, homebound older adults with long-term care needs to remain safely and independently in the community. This position oversees a multidisciplinary team, ensures high-quality, strengths-based case management, and supports program operations, compliance, and staff development.

Key Responsibilities
Provide clinical supervision, coaching, and leadership to Case Managers and direct service staff.
Conduct, review, and monitor client assessments, reassessments, care plans, and case documentation.
Authorize services, monitor waiting lists, and coordinate care with community partners and referral sources.
Conduct individual and group supervision, performance evaluations, and staff development activities.
Manage team schedules, timesheets, caseloads, and support social work student field placements.
Monitor program performance, service quality, outcomes, and compliance with NYC Aging and agency standards.
Maintain accurate program records, reports, and documentation required by funding sources.
Conduct outreach, community presentations, and represent SHOPP at professional meetings and community events.
Assist with recruitment, onboarding, policy implementation, resource development, and continuous quality improvement.
Participate in agency meetings, trainings, and other duties as assigned.
Qualifications
Master's Degree in Social Work (MSW) from an accredited institution required.
Experience in social services, preferably in community-based case management with older adults.
Strong clinical assessment, supervision, leadership, communication, problem-solving, organizational, and team-building skills.
Knowledge of aging services, community resources, and person-centered, strengths-based practice.
Bilingual Spanish/English preferred.
Proficiency in Microsoft Office required; experience with NYC Aging Peer Place or similar case management systems preferred.
Work Schedule & Requirements

This hybrid position requires a minimum of two in-office days per week, travel throughout the Bronx for community meetings and fieldwork, and attendance at required in-person meetings, trainings, and agency events. Occasional evening or weekend availability is required to support program activities.
