Position Summary

The Case Manager supports homebound older adults in maintaining independence, enhancing quality of life, and reducing social isolation through person-centered, strengths-based case management. Responsibilities include conducting comprehensive in-home assessments, developing care plans, determining eligibility for supportive services, coordinating referrals, providing advocacy, monitoring service delivery, and connecting clients with community resources.

The Case Manager maintains an assigned caseload, conducts routine home visits, responds to crises, provides client education, documents services accurately, and collaborates with clients, families, healthcare providers, and community partners to ensure quality care. Additional responsibilities include participating in outreach activities, program presentations, team meetings, training, and assisting with intake coverage as needed.

Qualifications

Bachelor's degree in Social Work (BSW) from an accredited institution required. Experience in case management or a social service setting is preferred. Knowledge of aging services, public benefits, and community resources is highly desirable.

Candidates should possess strong assessment, communication, advocacy, care coordination, problem-solving, organizational, and documentation skills, along with the ability to work independently and collaboratively within a multidisciplinary team. Bilingual Spanish is preferred. Proficiency in Microsoft Office is required or similar electronic case management systems is a plus.

This position follows a hybrid schedule and requires travel throughout the South Bronx to conduct in-home visits. Candidates must be able to travel independently, climb stairs when necessary, transport materials weighing up to 5 pounds, report to the office 2–3 days per week, and attend required in-person meetings, training, and community events

Skills
Strong assessment, communication, and organizational skills with the ability to work independently in the community. Experience developing person-centered care plans, coordinating services, and connecting clients to community resources. Ability to identify safety concerns, maintain accurate documentation, build rapport, advocate for clients, and demonstrate professionalism, empathy, cultural sensitivity, and sound judgment during home visits.
