Principle Responsibilities & Position Purpose: The Outcomes Manager under the direction of the Director of Employment Programs is responsible for the supervision of the outcomes coordinators and community coaches and will oversee participant issues within the IECBS Program. The position is responsible for all phases of consumer services for a specified case load including Person Centered Service Plans, participant case notes, case conferences, discharge summaries, etc.; is also responsible for the coordination of all services provided including consumer programming, issues, rights, advocacy, and volunteer opportunities in order to be in compliance with all regulations including WIOA and HCBS.
ESSENTIAL FUNCTIONS
Acts as liaison with community, representing the agency in a professional manner.
Oversees the IECBS orientation/training for program participants following the Anthesis program description and agency policies.
Provides information upon request regarding program evaluation of goals, objectives, and measurements of rehabilitation services.
Observes, records, evaluates, designs, implements, and monitors Person Centered Service Plans for each participant on assigned case load. Instructs and supervises the appropriate personnel to assist in implementation of the Person Centered Service Plans.
Works in collaboration with the Outcomes Coordinator and Community Coaches to perform job duties in determining appropriate, integrated, community-based activities and appropriate planning for each participant during program hours. May fill in as direct support staff to meet the needs of participants.
