Roles & Responsibilities:
To coordinate clinical services of patients. To be cognizant of the philosophy, standards, objectives and policies of the department and the hospital.
Provides community outreach and coordination crisis intervention services in accordance with psychiatric and psychosocial assessment and diagnosis.
Coordinates with other members of multi-disciplinary team in developing and implementing crisis intervention plan.
Outreaches team leader supervising Assistant Clinical Coordinator.
Documents in medical record diagnostic impressions of patient status and current progress. Provides education and outreach to the community on Mobile Crisis services.
Assists patients in bridging from CPEP to clinic.
Bridges community high risk referrals to CPEP as needed based upon assessment.
Submits provider reports in a timely manner.
Completes encounters on a daily basis.
Participates in the development of other staff members.
Meets regulatory, licensure and annual health assessment requirements.
Identifies learning strengths and needs.
Utilizes learning resources.
Maintains appropriate work statistics and other data.
Performs all other activities as assigned/approved.
Demonstrates a professional, courteous, and respectful attitude in dealing with patients, families and significant others.
Display courtesy, tact and patience during interactions with all members of the hospital staff and extended community.
Cooperates with physicians, administrators, co-workers in working towards departmental divisional goals and objectives.
Acts as resource for other mental health professionals.
Supervises or teach other mental health professionals and trainees.
*After 6 months of employement there will be a retention bonus provided.
