The CARES Coordinator is responsible for coordinating and implementing vocational rehabilitation services for individuals recovering from substance abuse.Essential Responsibilities:
Coordinate the referral process to Gateway Hudson Valley’s Chemical Abuse Rehabilitation and Employment Services (CARES).
Serve as a liaison between CARES, state & local government agencies, referral sources and other community agencies.
Market CARES. Provide outreach to potential consumers and referral sources.
Conduct structured interviews with all new referrals. Identify individuals’ work readiness status.
Collaborate with individuals and treatment providers to develop service plans.
Facilitate the career exploration process; assist individuals in setting short term and long term employment goals.
Assist individuals in identifying employment barriers and the services/skills necessary to obtain and maintain employment.
Develop and implement curriculum on job search skills, work culture, life skills and vocational coping skills. Present curriculum in individual and/or group sessions.
Coordinate internship activities.
Develop communication and contact between employers and job seekers by means of site visits, speaker’s programs and customized training presentations.
Monitor individuals’ participation in job search activities.
Assist individuals in locating employment opportunities.
Provide case management services to program participants during their job search and first six months of employment.
Provide vocational services in cooperation and coordination with clinical staff.
Represent CARES on county and interagency committees.

Participate in job development team meetings and case conferences.
Collect, maintain and report on program statistics.
Develop work-scope objectives; monitor and report on achievement of objectives.
Maintain required documentation; review case records for quality assurance.
Promote and maintain good public relations with the business community and develop and implement projects with employees.
Counsel participants in areas related to successful compliance with program guidelines.
Provide transportation to consumers, as needed.
Attend all scheduled training and supervisory sessions.
Maintain a work environment in a clean, organized and safe manner.
Conduct daily inspections of immediate work area(s) to look for unsafe conditions.
Ensure all accidents are promptly reported to immediate supervisor and Human Resources.
Be familiar with the Safety and Health Accident Prevention Program and abide by all safety and health rules, work practices, and regulations.
Responsible for understanding and adhering to Gateway Hudson Valley’s Corporate Compliance Policy and Standards of Conduct as well as all applicable program rules, regulations and policies.
Demonstrate and model positive work attitudes and habits for individuals.
Perform other duties as assigned.
