The Patient Navigator helps ScanVan patients communicate with their healthcare providers, so they get the information they need to make decisions about their health care. The Patient Navigator will provide case management and navigate those patients with abnormal breast, cervical, or colorectal screening results through the diagnostic process with PRI partner facilities and other providers as appropriate. The Patient Navigator will report to the ScanVan Director and Assistant Director.

Essential Duties and Responsibilities:
The essential duties of the Patient Navigator include but are not limited to the following activities:
•        Triage follow-up patients to contact the most significant abnormal patients first to discuss next step options and respond to patient questions and concerns
•        Document all follow-up notes and conversations into ECW to ensure patient compliance and to record all recommendations
•        Scan all applicable documents into patient ECW chart, to include written correspondence, mammography reports, radiologist interpretation reports and patient letters
•        Follow up with PCP to report abnormal results and need for follow up (when applicable)
•        Determine appropriate funding source for patient (health insurance or grant funding)
•        Arrange appointments with an appropriate participating provider within the insurance network for diagnostic exams where applicable
•        If the patient is uninsured arrange an appointment with an appropriate Cancer Services Partnership, (CSP), or Breast Treatment Task Force, (BTTF), provider
•        Send patient images, radiological, laboratory, intake (CSP or BTTF) reports and consents to the appropriate party (Patient, PCP or Diagnostic Provider) for breast follow up when needed
•        Communicate detailed patient follow up appointment information
•        Generates daily radiology reports and patient outcome letters
•        Request follow-up reports from the diagnostic providers and follow the patient through the diagnostic process
•        Assist patient in arranging additional appointments with other providers as needed.
•        Update ECW to reflect all final diagnosis information and patient status
•        Send certified letter to those patients who have refused follow up treatment
•        Generate reports and letters as needed
•        Answers ScanVan phones and schedules appointment
•        Attends meetings as requested

•        A minimum of one year breast care patient navigation experience
•        Ability to work independently with minimal supervision
•        Organized and detail oriented
•        Computer literate – Microsoft Word and Excel knowledge a must
•        Knowledge of electronic medical records
•        Must be able to handle large volume of work
•        Must be able to prioritize
•        Associates degree (required) Bachelors preferred
•        Nursing or Social Work education a plus (medical assistant)
•        Excellent Communication Skills
•        Bilingual; fluency in Spanish preferred
