Under the supervision of the Chief Program Officer, this position is responsible for the development, planning and implementation of the Supportive Care Programs of the agency, which include the daily training and oversight of Health Homes care management services, Harm Reduction Services, Social Networking Strategies and Linkage Navigation and Retention services for HIV Infected/affected individuals. Oversight of a 340B pharmacy program. This position provides key strategic direction and develops department-wide policies; ensures sound implementation of programs, initiatives, and change to adapt, improve, and augment trauma sensitive quality of care in a changing health care environment; develop program budgets; develops and implements compliance processes; conducts total quality improvement projects to improve services and define best in class practices; evaluate and determine work flows and staffing patterns, conducts grant writing and procures funding to align with agency’s mission; leads the agency to trauma-informed certified entity.
•Contract negotiation, contingency planning, and reporting
•Grants writing and program procurement. Identify partners in providing services to ensure an integrative model of care.
•Act as liaison for the agency with regulatory entities such as Department of Health (CLIA Waiver)
•Develop protocols and processes for evaluating the HIV testing programs and monitoring procedural and regulatory activities; train and monitor staff to perform HIV testing procedures.
•Assessment of needs, planning, coordination and implementation of health services which includes, AI Case Management, the Health Homes programs, Harm Reduction Services, Social Networking Strategies.
•Is a member of the multidisciplinary, healthcare team for the health homes and harm reduction services and communicates and interacts with all levels of staff, primary care providers and clients.
•Responsible for the development and maintenance of policies and procedures for the Social Support service department.
•Participates in the development of the budget, program and business plan.
•Serves as field supervisor for students in field placement/internship.
•Participates in the quality improvement program that will assure that clients receive quality care in the office and from other providers of health care services, using peer review, Chart record audits, grievances and other mechanisms as appropriate.
•Adheres to the standards and policies of the Organizational Privacy/Security and Compliance Programs, including the duty to comply with applicable laws and regulations (HIPAA, OSHA, OMH, OASAS, guidelines, and other State and Federal laws). This also includes reporting to the Board of Directors, Compliance Officer, Privacy Officer, supervisor or suggestion box, any suspected unethical, fraudulent, or unlawful acts or practices
•Supervises Managers/supervisory staff. Follows standards for progressive disciplinary action.
•Completes monthly departmental reports
•Updates QA & Key performance indicators
•Knowledge of the health workforce and/or HIV/AIDS related service experience.
•Demonstrated progressive advancement in managerial/administrative position, particularly in the healthcare field.
•Ability to effectively present information both orally and written to administration, the public and/or Board of Directors.
•Ability to work with a culturally diverse group of people.

EDUCATION :Master’s degree in health related field from an accredited college or university, or B.A. and 5 years’ experience in the field, Plus supervisory skills and/or experience
Prefer: MSW with SIFI

•        Must be proficient in Health Homes, evidence-based interventions and performance-based program designs.
•        Demonstrated progressive advancement in managerial/administrative position, particularly in the health related field.
