The Family Peer Support Service Worker assists the program in providing services to families in the areas where support and advocacy is needed, but do not require the full services of the Care Manager. Duties include escorting, advocacy, assisting families to access community resources, and family support. Accompany and transportation of children and families are essential components of the position.   
        ESSENTIAL DUTIES        
•        Comply with New York State Office of Mental Health Regulations
•        Assist families to become independent and be prepared for transition and termination of services, assist in the intake and discharge processes of families
•        Home visitation
•        Accompany families to appointments to serve as a bridge between families and service providers
•        Undertake activities such as advocacy and referral to facilitate clients to service needed, including but not limited to education, health and mental health services, day care, housing, vocational guidance, employment, financial entitlements, immigration, recreation/cultural activities and foster care/residential placements
•        Coach and model shared decision-making and skills that support collaboration, in addition to providing opportunities for families to self-advocate
•        Providing emotional support for the family on their parenting journey to reduce isolation, feelings of stigma, blame and hopelessness
•        Completion of required documentation within allotted time, utilizing the computer.
•        Attend job-related in-service training sessions annually
•        Attend all required programmatic meetings
•        Accompany Care Manager to client’s homes as needed
•        Create a weekly schedule involving assignments approved by the Program supervisor.
•        Coordinate and implement support groups activities for parents
•        Provide resources, including but not limited to education, training, advocacy, and support.
•        Attend Family Peer Support Services Credentialing training

•        Demonstrate ‘lived experience’ as a parent or primary caregiver who has navigated multiple child serving systems on behalf of their child(ren) with social, emotional, developmental, health and/or behavioral healthcare needs.
•        A high school diploma, This educational requirement can be waived by the State if the person has demonstrated competencies and has relevant life experience sufficient for the peer credential.
•        Requires a valid, non-restricted Driver License
