       Under the direct supervision of the Outreach
       Coordinator, the Outreach Health Educator
       conducts outreach and provide group and
       individual services to persons at high-risk to reduce
       the transmission of HIV/AIDS and Hepatitis C.


a.           Conduct street outreach to active substance
          users who are at high risk for HIV/AIDS & Hep C.
b.           Perform group/individual counseling sessions to
          access clients needs.
c.           Provide clients with current accurate health
d.           Assist in project data collection, evaluation
          activities, and required reporting. Complete all
          required documentation accurately, in a timely
          manner, and thoroughly in accordance with
          departmental standards and program/grant
e.           Maintain accurate and complete field notes and
          logs including number and type of contacts
          made, demographic information about the client,
          materials distributed, health education or group
          sessions conducted and number of participants
          in a timely manner.
f.           Distribute and provide access to clean syringes
          in street-based and clinic-based settings
g.           Provide clients with supportive services i.e., HIV
          testing, STI & HEP C screening and linkage to
          care for all persons who test positive, referrals to
          SUD treatments, housing, medical and mental
          health, transportation etc.
h.           Facilitate an evidence-based curriculum “Safety
          Counts” incorporating various learning
            techniques and formats (i.e., group discussion,
            lectures, role-play, etc.). Review and assist in
            determining the linguistic and cultural
            appropriateness of materials, teaching aids,
            activities, methods, strategies, programs, etc.
            Review the efficacy of materials and programs,
            and provides positive, action-oriented feedback.
i.           Provide Overdose Recognition and Response
          Trainings (Naloxone) to community members and
            program participants
j.           This position requires flexibility and ability to work
          early mornings, late afternoons, weekends and
          some holidays as needed in order to best assist
          clients needs.
k.           Perform other duties as assigned.

a.        Two years of college education related to field or
       equivalent work experience related to field.
b.        Permanent resident or U.S. Citizen; proof of work
       eligibility upon hiring.
c.        Valid California Drivers License and current
       automobile liability insurance.
d.        Must be able to work with people of diverse
       cultural, educational, social, and economic
e.        Demonstrated knowledge and skill in computer

f.        Recent physical examination and TB, Covid test
       and Complete Covid Vax necessary upon hiring.
g.        Physical health that allows the employee to walk
       distances up to 1 mile several times a day and
       work there assigned tasks for periods up to 5 hours
       per day. Duties may occasionally include lifting up
       to 40 pounds, climbing a flight of stairs and light
       cleaning of the office area.
