Project FIND’s mission is to provide low- and moderate-income and homeless seniors with the services and support they need to enrich their lives and live independently. Today Project FIND operates three supportive housing residences that are home to about 600 people and four senior centers with over 3,000 members. Our members and residents range from the healthy and active to the frail and homebound to the homeless. For all these men and women, Project FIND is a critical resource, providing housing, meals, and programs that help individuals navigate the challenges of aging by encouraging community engagement and healthy living.

General Responsibilities
This position provides social services to the tenants in Project FIND housing programs. Social workers are embedded in the programs to provide support to tenants that will help them thrive. Day to Day work includes helping tenants connect with the community, obtain or restore entitlements, connect with family, combat isolation and obtain health care services. As a housing social worker, many times the task is to act as an extension of property management helping tenants with payment of rent, recertification, one-shot deals, connections to APS, home health services, and more. 
Job Summary:

Responsible for assessing clients' needs and providing casework services to tenants. This will include case management, information and referral, short-term counseling, and crisis intervention.
Administers the outreach program to tenants at Hamilton/Hargrave House which involves meeting with tenants and filling out a Tenant Information Form with each tenant.
Keeps case records on tenants and submits required statistics and reports on work done.
Works closely with the Building Manager regarding the social service needs of tenants.
Works with the Visiting Nurse Service and any other health providers to ensure that appropriate, coordinated nursing and social services are provided to tenants.
Develops linkages with community agencies to enhance the delivery of social services.
Works with the Social Work Team to ensure that tenants, who are also senior center members, are referred to appropriate social or medical services as the need arises.
MSW,LMSW preferred.
2 years experience working with the elderly, preferably in housing or congregate setting.
Computer literate.
