Title:                 Clinical Coordinator
Program:          Fort Washington Men’s Shelter

Overall Responsibilities:
Under the general Direction of the Shelter Director, with the latitude for independent action and decision making, the Clinical Coordinator is responsible for the overall coordinator and delivery of services to clients assigned to the on-site and rehabilitation program funded by DOHMH. The Clinical Coordinator workers collaboratively with the Shelter Director and other shelter leadership to ensure the on-site rehabilitation staff provides high quality case management and housing placement services to clients. In addition, the Clinical Coordinator, directs staff in providing assessment, counseling, and referral services to the clients participating in Team PRIDE and Team STEPS initiatives, ensuring the needs of clients are being met and program objectives are being achieved.
Program Description:
Fort Washington Men’s Shelter is a 200 bed 24/7 emergency shelter for homeless men diagnosed with mental illness and/or substance use disorders. Our comprehensive services include case management, individual and group counseling, recreational activities, and housing placement assistance. Additionally, an on-site Article 28 Clinic provides primary and behavioral health care services.
Essential Duties and Responsibilities:
The essential duties of the Clinical Coordinator include but are not limited to the following activities:
•        Day to day supervision of programming and service delivery to 130-135 clients
•        Provide clinical and task supervision to a staff of 1 Senior Case Manager, 6 Case Managers, 1 Intake Coordinator, and 3 Engagement Specialists
•        Review and sign off on documentation completed by the case management team
•        Oversee the data in the DHS CARES system, ensuring that the data is current and that case notes, ILPs, psychosocials, and referrals are being provided by the team
•        Facilitate case conferences with clients and case managers
•        Facilitate weekly team meeting with case managers and housing coordinators
•        Maintain and report statistical data for DOHMH and DHS
•        Conduct chart audits
•        Train new staff and attend trainings as assigned
•        Perform other duties as assigned by Shelter Director
•        Ability to be on call as needed
Education Requirements
1.        MA in social work, counseling, psychology, family therapy or related field, plus 1 year supervisory experience and 2 years’ experience working with individuals with mental illness and homelessness OR
2.        BA in a related field with 2 years supervisory experience and 4 years’ experience working with individuals with mental illness/substance use and homelessness
Computer:         Knowledge of case management software as well as proficiency in Microsoft Office Suite. Understanding and familiarity with CARES (DHS Data Base) a plus.
Language:           Bilingual or multilingual a plus.
