Description will supervise Outreach case management staff, including clinical supervision. Engage and provide services to homeless individuals, ensuring contract level of services are met. Track incident reports and implement quality service plans. Liaise with communities per Director's direction. Develop staff training and monitor placement reports, progress notes and chart data. Assist with DHS report, housing packets, chart auditing, and stabilize bed list. Manage canvassing and 311 calls. Total of 333 characters. | ||||||||||||||||||||

Skills Requires a Bachelors degree, Master’s degree, preferably LMSW or LCSW Pref'd. EXP in the fields of homeless services, street outreach, substance abuse and/or mental health treatment. Min of 2 years Administration &/or Supervisory Exp. Process exceptional oral and written communication skills. Clean Drivers License | ||||||||||||||||||||
