Description
SUMMARY:   
Responsible for managing the clinical, administrative and supervisory functions of the HOPE Housing Program, an OASAS licensed supported scatter-site apartment program for single adults and/or families. Clients in this program all have a current or past diagnosis of substance misuse as well as a history of homelessness.                        

RESPONSIBILITIES:

Oversee a roster of clients with the greatest risk of recidivism into homelessness and relapse
Oversee the completion of housing-related needs assessments and the development of service plans clearly identifying goals, objectives, outcomes, and timeframes for completion
Participate in lease negotiations for program apartments in conjunction with the Operations Team and Associate Area Director
Responsible for budget planning, spending, rental collections from clients and outside agencies and monitoring in compliance with contract and agency standards
Conduct weekly individual supervision sessions and regular Team Meetings with program staff
Preparation of all relevant program reports and data collection; including analysis and reporting
Provide crisis intervention as required
On-Call with 24-hour cell phone coverage
Oversee the maintenance of confidential and accurate client charts and documentation
Act as liaison with other PH programs and community agencies for the care coordination of the clients

Skills
Position requires a Bachelor’s Degree, an MSW/MHC Degree a plus
Experience in the housing field along with experience working with the homeless, substance use and mentally ill populations preferred
Supervisory skills required with program oversight experience needed
Need to establish workload priorities and balance diverse duties
Must have excellent communication skills, both orally and in writing, regarding complex and sensitive issues
