Description
PURPOSE To serve in the supervisor role as an active advisor, consulting with the Social Work staff. This includes assessing each program, developing and revising the scope, mentoring, reviewing policies and procedures of each program, and monitoring performance metrics. The supervisor possesses the ability to see patients and for coverage or in times of high census.ESSENTIAL FUNCTIONS *Responsible for clinical and administrative supervision of the social work staff for the Hospice House and In Home Hospice programs. o Clinical supervision of Social Workers includes discussion of all new patients, clients, or community members and their families; any problem situations or need for family meeting; and any crisis situation that may need to be reported to the Adult Protective Services or Child Protective Services or other agencies. o Administrative responsibilities includes reviewing and signing time cards, approving vacation or other time away from the agency, yearly performance reviews, keeping the social services staff aware of management decisions, and any disciplinary actions. Monitoring performance metrics as established for each program. *Conduct bi-weekly or weekly meetings with social work staff to inform staff of management decisions, to assist with staff clinical issues, to provide clinical supervision, and to involve staff in planning activities. *Responsible for overall planning, delivery, and evaluation of quality social work for In-Home Hospice and Hospice House programs. *Assign social worker for each patient intake/informational and assure that there is ample coordination with the Clinical Nursing Supervisor and the RN Case Manager. *Review charting completed by Social Workers to ensure that there is: o Adequate and timely assessment of the patient and family needs, as appropriate, or defined by policy, and that information is shared with the interdisciplinary team members. o An individual plan of care for the patient and family is developed in collaboration with the interdisciplinary team members and approved per protocol. o Documentation of their activities in accordance with established record-keeping procedures and agency requirements. *Communicate with the Director of Spiritual and Community Care to keep him/her/them fully informed of activities, decisions, and plans; and assemble, prepare, and submit reports as required or requested. *Act as a resource for social service personnel in the assessment of the patient/family's knowledge of psychosocial issues as they relate to their care needs. *Identify training and educational needs of Hospice Social Workers. *Assist the Director of Spiritual and Community Care to develop and implement the Social Service program's training plan. *Participate in community education and public relations programs as requested by the Director of Spiritual and Community Care and/or the Chaplaincy Health Care Chief Executive Director.

Skills
QUALIFICATIONS *Graduate of an accredited school of social work; Master's degree in Social Work required. Accredited by the CSWE. *Experience in program development, coordination, and management (preferred). *A minimum of one year experience in medical/healthcare social work (hospice preferred). *Active membership in the local, state, and national professional social work organization *Available to work occasional evenings, nights and weekends as part of the on-call administrator rotatio
