Project Renewal’s Re-Entry Permanent Housing provides supported housing to 25 men/women with serious and persistent mental illness/dual diagnoses – who also have a history of significant criminal justice involvement/may be under parole supervision. The program directly delivers and/or facilitates access to a broad range of services geared towards clients’ stabilization and community integration.Currently, we are seeking a Forensic Housing Case Manager at The Parole Support and Treatment Program, will manage a caseload, provide referral for services (medical, substance use, medical etc.), develops service plans, complete intakes, psychosocial, case notes, facilitate group etc. Makes face to face contact with each client at a minimum 2 times each month. A Degree (Masters strongly preferred); significant experience with homeless population, excellent communication skills; experience with Microsoft office. Must be fully vaccinated against Covid-19.
Salary: $38,000-$42,000 a year
Schedule: HYBRID WORK POSITION (3 days in-office; 2 days remote)/ with visitation at clients homes
