Description
LETC - Carl Fenichel Community Services program is seeking an Intake Coordinator to handle recruitment and intake within the Adult Services program; which includes Day Hab (DH), Community-Based Prevoc (CBPV), Supportive Employment (SEMP), and ACCES-VR employment services. Responsibilities include but are not limited to: • Recruits participants for all day services programs by networking with partner agencies, including Care Coordination Organizations (CCOs), School Transition Coordinators, OPWDD, self-advocacy groups, DD councils, families, and other entities. • Responsible for maintaining program enrollment in adult services programs. • Networks with referral sources to promote new referrals and growth. • Coordinates intakes process: including collection of referral packets; scheduling pre-placement meeting and onsite visits and trials; clearly communicates with all members of the person’s support team. • Responsible for complete, accurate and timely submission of all documentation, referrals, follow ups and verifications per agency intake process. • Assists with eligibility for OPWDD services (Medicaid and OPWDD eligibility) in partnership with Care Managers, Transition Coordinators, and/or family members. • Maintains master intake management tracker (waitlist) and generates reports to management monthly. • Ensures collection of all required documentation for purposes of billing and compliance. • Conducts internal quality reviews in partnership with QA and management. • Complete Psychosocial Assessment as needed. • Manages agency rosters via CHOICES Education and experience: • Bachelor’s Degree in social work, psychology or related field (required); Minimum of 3 years’ experience working with I/DD or related social service experience. • Master’s Degree in social work, psychology or related field (preferred); one year experience working with I/DD or related social service experience preferred but not required. Skills Required: • Excellent communication skills, both oral and written; strong presentation and networking skills; strong advocate for people with disabilities. • Must be a team player and have strong work ethic. • Proven ability to cultivate and maintain employer relationships with business and industry with a particular focus on employers providing employment opportunity in various fields.For immediate consideration, please email your resume with salary requirements to employment@leaguecenter.org.
