Job Description: Come join our team! St. Francis House is looking for part-time LMSW -HCHV (Health Care for Homeless Veterans) Case Manager. St. Francis is a transitional housing program for homeless Veterans while they work towards permanent housing.The HCHV Case Manager will be responsible for providing case management to the Veterans residing at St. Francis House.
This position is part time (30) hours weekly, Monday - Friday. Hours are 4:00pm - 10:00pm or 3:00pm - 9:00pm. We are hiring immediately for this position. This is an excellent position for great pay!
Weekend call may be necessary when needed.
Job Responsibilities: The HCHV-MSW Case Manager will be responsible for:
Weekly individual progress meetings with the Veterans residing at St. Francis House.
Facilitating weekly therapeutic groups.
Maintaining case files that document the progress of the Veterans in the transitional housing program.
Transporting Veterans to appointments (if needed, utilizing company vehicle)
Being an active participant in interdisciplinary team meetings for the veterans
Provide crisis intervention as needed
Conducts admission screening, provides consultation, education, and referrals for Veterans regarding community resources for VA services and benefits.
Administering drug tests and implementing VA protocols.
Working with the VA Day Treatment Center to ensure Veterans are on track with meeting their Individualized Service Plan goals.
Other duties as assigned by management
Qualifications:
Masters' degree in social work
LMSW license
2 years of case management experience (preferred)
Experience working with homeless veterans (preferred)
A willingness to support our Veterans and work to ensure their positive outcomes
The ability to work independently with diverse populations
