Description
STRIVE is a new program in partnership with Metropolitan Center for Mental Health and the NYC Department for the Aging. STRIVE’s mission is to foster Support, Treatment, Recovery, Independence, Voice and Empowerment for older New Yorkers who need it the most. STRIVE also seeks to provide education for providers and organizations working with older people to promote acceptance and community inclusion. STRIVE is a model of community engagement that aims to close gaps in services and supports for older adults with a lived experience of mental illness and/or substance use.The STRIVE Social Worker will use a comprehensive, whole-person approach to support wellness and healthy aging in place for older New Yorkers. This position will involve extensive collaboration with program partners, community-based assessment to facilitate linkage to services and supports, outreach, education, and data collection/ tracking. The successful candidate will have a passion for direct service with adults, and an ability to be a strategic thought partner to inform program design. Previous experience supporting individuals with mental illness, substance use and/or case management needs is required. LMSW (or other licensure) and previous experience with electronic documentation is required. Bilingual Spanish-speaking candidates are highly encouraged to apply.
