Neighborhood Self Help by Older Persons Project (SHOPP) is a non-profit social services agency located in the Bronx serving minority older adults. SHOPP’s mission is based on the philosophy of self-help. We believe that older people are capable of helping themselves and helping others improve their capacity to deal with the physical, emotional, social, and financial challenges they face as they age.The purpose of the Neighborhood SHOPP Case Management Program is to provide services to the frail elderly with long-term care needs who wish to remain in the community. The Case Management Program is a 100% DFTA-funded program and employment is contingent upon successful contract renewal. Utilizing a strength-based approach, Case Management objectives include making an appropriate determination of client needs, strengths, abilities, and interests; linking clients to services and resources in the community, and monitoring and conducting reassessments to ensure appropriate service utilization.
Job Duties:
The Friendly Visiting Coordinator is specifically responsible for the following:
· In collaboration with SHOPP administration, establish principles, procedures and an action plan for volunteer outreach, screening, client eligibility, assessment, service initiation, monitoring, reassessment and termination;
· Maintain program records, and reports required for documentation and statistical reporting for funding sources;
· Conduct outreach, informational and program presentations to community groups for volunteer recruitment purposes.
· Recruit volunteers from interested local residents to visit with CMP clients.
· Screen and interview prospective volunteer candidates, conduct reference/background checks;
· Provide orientation and training on the needs of the homebound elderly and emergency protocols; identify preferences and interests and availability.
· Make a match between the FVS volunteer and CMP client; initiate introduction process;
· Monitor and evaluate volunteer assignment through FVS volunteers via telephone calls/emails and with clients via in-home visit and telephone calls;
· Develop and maintain a team approach volunteer corps structure that enables peer interaction and support;
· Conduct ongoing acknowledgment of volunteer efforts and hold an annual volunteer recognition event;
· Engage CMP staff in the identification and referral of potential clients for FVS.
· Conduct initial telephone screening with potential client to provide information, identify interests/preference and confirm consent, formally accept a client for FVS;
· Maintain communication with CMP staff re: FVS status, issues, or concerns;
· Assist in other agency duties as assigned.
