Position Title: Clinical Case Manager

Division: Employment Opportunity Center (EOC) EOC is a program designed to equip and empower young adults (ages 18-29) to pursue employment and career goals through paid work opportunities, coaching, supportive case management, and career exploration. Case managers directly provide or connect clients to resources that provide food, clothing, legal assistance, transportation, therapy services, etc. Services provided directly relate to helping the participants in the program stabilize his or her life, secure and maintain employment, and establish independence as young adults. EOC participants are also enrolled in a Career Development course designed to help them develop or improve upon appropriate employment behavior, communication, decision-making, job searching skills and increasing personal assets while minimizing existing barriers that impact their ability to be successful at work.

Job Responsibilities: The Clinical Case Manager is responsible for assessing, coaching and serving 15-18 EOC participants to become successful employees. Responsibilities include full assessment and case planning, individual meetings to set goals and provide accountability and support, life skills instruction, job counseling, ongoing communication with community partners, maintaining case files, generating reports and transporting clients as needed. Additional responsibilities include working with the team to ensure case management services are successfully provided to all participants, taking the lead on mental health consultation, and working with participants placed with Corporate Partners to ensure success. This position reports to the Employment Opportunity Center Program Coordinator.

Requirements: Master’s degree in Social Work, Counseling, Human Services or related field with 1-3 years of previous clinical/case management and youth or young adults experience. Demonstrated skill in assessment, engagement and collaborative problem solving with clients. Knowledge of public systems and resources. Demonstrated sensitivity to the needs of low-income and culturally diverse populations. Proficiency in Microsoft Word and Excel. Strong organizational and communication skills. Valid IL driver’s license, good driving record and reliable vehicle. A commitment to OCM’s Statement of Mission & Faith.

Status: Full-time at 40 hours/week. Schedule is Monday – Friday and includes one evening each week to meet client needs.

Compensation: Annual salary falls between $40,000 - $44,000 based on education and experience. Licensed supervision available.
