Description
The Bilingual Case Manager is central to the CCBHC; ensures clients receive the clinical services needed to support their recovery. On the clinical team, provide Targeted Case Management to support recovery of CCBHC participants via supporting client engagement in services, ensuring care coordination of clinical treatment services and concrete and social support needs of the client and the family, and providing crisis support as needed. Responsible for ensuring the accuracy, timeliness, and integrity of all charting (EHR/AWARDS) and service data requirements of the Federal Substance Abuse and Mental Health Services Administration (SAMHSA) ICS Family Life Center CCBHC grant, and the article 31 and 28 clinics. This position is federally funded and as such COVID vaccination is required with very rare exceptions based on religious or medical exemption. Duties: Provide targeted case management to support client’s recovery and prevent crisis with Illness Management and Recovery Evidence-Based Practices; Support client engagement in clinical services with phone outreach to clients (and family of clients) who are consistently missing appointments; outreach home visits as needed; make referrals to providers to meet the concrete needs of clients, i.e. enrollment in benefits and entitlements, insurance, housing, advocacy, career counseling, etc.; update pharmacy, insurance, and discount cards for medication management; referrals to Prevention services (i.e. suicide prevention, SU prevention, parenting skills, etc.); Conduct crisis prevention via outreach and engagement into clinical treatment and center’s social and family support services; Make home visits in emergent situations during clinic operating hours 9 a.m. to 9 p.m. Monday through Saturday; For a client experiencing a crisis, assist in the facilitation of a “warm hand-off” between ICS Family Life Center CCBHC outpatient services and ER/ED (whether at client’s residence or on-site at center); Cover the project’s emergency phone line during program off hours, 9pm to 9am, weekends, and Holidays, with telephonic intervention via 12-hour shifts, on a rotating basis. If the situation is resolved/not an emergency, but is an urgent situation then schedule the client to come in the following day to meet with a clinician (ideally their therapist, otherwise an available clinician). If the patient is in imminent danger the case manager calls 911/EMS and interact with the EMS personnel and the client via cell and video to help guide the process and calls the ER ahead of time to announce the arrival of our patient; Mobile crisis teams through NYC Well used as needed for urgent home visits during unforeseen circumstances (i.e. inclement weather, etc.). Crisis management is a developing service in light of the upcoming national 988 system; other assignments as per the changing needs of the ICS’s CCBHC. Abide by HIPPA and clinic standards of confidentiality; Participate in required training.

Skills
Qualifications:
Associates, Bachelor’s, Masters degree or comparable experience required
Minimum of two (2) years in a behavioral health setting
Must be highly organized and detail oriented with strong ability to multitask
Excellent interpersonal and communication skills
Must be computer literate, and proficient in Microsoft Office
Bilingual Spanish is required.

Additional on-call: $15/hour 9pm to 9am shifts; major medical, dental, retirement, life insurance, annual and sick leave, holidays; Duties (continued): client virtual set-up; Support clients and family with illness management using MI, person-centered, trauma-informed, and psychoeducation approaches (i.e. orientation to the CCBHC program, signs and symptoms, coping strategies) ; conduct social activities for clients and families, assisted by program peer & support staff
