Job Summary

The Family Support Specialist collaborates with the Family Team Facilitators to support the families served within the Lake County Wraparound Program. This role advocates for the families, ensures access to community resources, provides transportation and conducts home visits. The program uses the Wraparound process, which is a family-centric model, which places the family at the center of decision making. The Family Support Specialist assures care is delivered in a manner consistent with strength-based, family-centered, and culturally competent values. Families in this program are involved in the Child Welfare system or are at risk of becoming involved. The expectation is that the individual within this role has experience accessing resources in the community, involvement in the child welfare system, and/or was previously involved in the Wraparound program.

Essential Duties and Responsibilities

Utilizes effective interpersonal skills to engage families and community stakeholders.
Provides in-home supportive services to children and families.
Provides emotional support to families in crisis.
Works cooperatively with other agencies, caseworkers, and stakeholders.
Helps families connect with community resources (counseling, housing, energy assistance, etc.).
Conducts home visits to check on family well-being and provide connection to the program.
Transports families to court, school, doctors’ appointments, etc.
Helps clients reach goals as described in their individualized Wrap Plans.
Supports the Family Team Facilitator and Caseworker.
Gathers and inputs data electronically for service progress tracking and quality assurance.
Adheres to the fidelity of Wraparound facilitation, as taught during on-the-job training.
Advocates for the family, especially around meeting their needs in the least restrictive setting possible.
