Description
Under the supervision of the Unit Supervisor the incumbent is responsible for the day-to- day provision of harm reduction mental services which consist of multiple concrete services within a psychosocial context, to include referrals, advocacy and crisis intervention. All responsibilities must follow Public Health Solutions and NYC DOHMH as well as, BAC Institute guidelines for HIV Postive Individuals.
Duties
•Serve as the primary Harm Reduction Counselor to HIV+ clients with a history of or present substance use.
•Conduct outreach and in-reach to clients and engage them in comprehensive risk assessment to determine match for HRM services.
•Work in a team model and ensure on-going contact with clients on caseload.
•Complete assessments, reassessments and service plans as specified by BAC and Public Health Services in a timely manner.
•Record all contacts and transactions with clients, collaterals, and service providers in the eCR.
•Conduct weekly group and individual sessions with clients.
•Meet contract deliverables per contract mandates.
•Develop a comprehensive service plan, including coordination of services and provider referrals using the SMART Goals method.
•Provide follow up for all client referrals, missed appointments, and missed individual/group sessions
•Provide supportive counseling to assist with implementation of service plans and encourage client self-sufficiency and empowerment.
•Monitor counseling services to ensure that services are provided, and client is engaged in care plan.
•Promote compliance with medical and social support appointments
•Attend meetings, trainings, and development seminars.
•Weekly Supervision and Team Meetings with HRM Supervisor.
•Other duties as assigned.

Qualifications: SKILLS, KNOWLEDGE, and ABILITIES REQUIRED:
•Motivational Interviewing Skills
•Case Management Assessment Skills
•Strong verbal and written communication skills
•Good interpersonal communications skills and the ability to empathize with, relate to and effectively work with participants and peers with chronic illness, substance use and mental illness, or a history thereof.
•The ability to relate to people of diverse backgrounds, cultures, races, sexual orientations and gender identities or expressions.
•Knowledge of community resources for people with chronic medical and behavioral health issues.
•Possesses excellent judgment and the ability to maintain professional relationships and boundaries when working with clients.
•Second language (Spanish, French and or Creole) a plus but not required
•Knowledge of HIV/PrEP and PEP
WORK EXPERIENCES
•Knowledge of issues related to the HIV+ population, substance use, recovery and mental health, PREP/PEP and overdose prevention counseling.

Skills
Education: BA degree in Psychology, Sociology, Social Work or other related field preferred or 3 years experience working in the provision of case management services to HIV+ or At-Risk population.
CERTIFICATION: CASAC or CASAC-T
