MAJOR DUTIES AND RESPONSIBILITIES
Develop and maintains meaningful, productive relationships with families.
Maintain enrollment throughout the program year through various recruitment strategies.
Recruit and enroll families into the program.
Participate in initial home visit with assigned Parent Educator
Assist parents with understanding and implementing the Family Partnership Agreement in order to encourage and promote their overall development, including achievement of self-sufficiency as well as positive developmental outcomes for their children.
Conduct periodic home visits and phone calls throughout the year to assess family resources and needs.
Identify services currently available and those needed by families enrolled in the Early Head Start program.
Develop relationships with other community-based service providers targeting parenting families.
Refer and link children and parents to needed services, including but not limited to (EI/LEA, WIC, housing, food pantries, medical clinics, etc.)
Work in collaboration with other SGA staff and consultants to monitor, track and coordinate services for children and parents.
Responsible for data entry and monitoring of child health, immunization, dental, disabilities, and community referrals utilizing the Childplus system.
Inform families when required physicals, immunization records, etc., are due and conducts home visits to pick up physicals.
In collaboration with the Parent Educator, implement protocols to follow-up on absenteeism.
Participate in staffing and/or family child reviews of all families on their caseload and case studies.
Maintain records and documents services in a timely manner.
Serve as an advocate for Early Head Start families and as a liaison between families, the program, and the large community.
Facilitate parent’s training and promote parents’ overall involvement and participation in the training.
Assist in planning and implementing parent activities during group socialization and encourage parent participation in program activities and obtain feedback from parents regarding the level of satisfaction with the services they receive.
Participate in ongoing training and staff development provided by Start Early, Family Support Coordinator, EHS & HS Director, other consultants, workshops, and conferences.
Attend regularly scheduled staff meetings, family support meetings, social service and health meetings.
Assist in identifying children with special needs and makes the necessary referrals.
Participate in the evaluation of Early Head Start services and ensures compliance with Early Head Start Performance Standards.
Contribute to the Agency’s efforts related to prevention and integrated service delivery with other community providers. Illustrates knowledge of the Agency’s Continuous Quality Improvement Activities.
Support and contribute to inter-system collaboration with other service providers to improve social service delivery systems.
Collaborate with Education staff to facilitate children’s transitions.
Collaborate with Disabilities Agencies to recruit children with disabilities into the program.
Collaborate with Health and Nutrition Consultant to ensure health and nutrition needs of children and families are addressed.
Complete other duties as assigned.
QUALIFICATIONS FOR THE JOB
_AA, AAS, BA, Advanced degree or credential or certification in Social Work, Family Services, Human Services, Counseling or in related field
