The Director of Case Management leads and supervises the client management team in all client engagement and case file maintenance, along with individualized treatment planning and implementation.
Responsibilities & Duties (Supervisory functions)
Collaborate with the Intake Coordinator to approve or deny former applicants requesting to return to the residential program; advise on other complex application issues
Determine caseload assignments based on client needs and Residential Case Managers’ expertise
Provide leadership and direction to all Case Management staff, to include weekly individual and group supervision meetings.
Oversee and ensure compliance with Guest House standards regarding case file maintenance
Serve as administrator for the client database system; oversee internal and external reports to ensure timely and accurate collection of data relative to residential, aftercare and outreach clients.
Regularly run reports to ensure the timely completion of required documentation (case notes, service tracking, etc.)
Assist with grant writing and reporting as required by Director of Marketing and Development
Maintain the safety regulations of facilities as mandated by DOC and Guest House
Approve schedule changes and requests for the Case Managers
Facilitate the social work internship program, training and onboarding for all Residential and Aftercare interns
Provide feedback and input on disciplinary actions for residents, as needed
Assist in organizing and scheduling training for case management staff
Assist in program development and restructuring
Serve as primary program liaison with Probation and Parole officials and the Community Services Board (CSB).
Provide intensive oversight of the Case Management team to include such case management duties as orientation for new residents within the first three days; new resident intake meetings and the development of Individual Service and Goal Plans for each new resident within 7 business days of their entry; establishing program protocols for financial literacy and budgeting skills for the residents; bi-weekly case notes and weekly progress notes as mandated by Guest House and the Virginia Department of Corrections (DOC) policies
Ensure that case files and other required documentation are maintained according to agency and DOC standards
Regularly identify and maintain relationships with new employment, housing, medical, and other community resources
Communicate regularly with relevant stakeholders as part of a resident’s treatment team (e.g., Probation Officers, Resident Specialists, etc.)
As needed, supervise and/or provide facilitation of psycho-social groups
