Job Summary: Sunnyside Community Services is seeking an Intake Specialist who will be a part of the Senior Services - Care Connections team who are the first to speak with prospective clients. As the Intake Specialist, you will help identify and make seamless connections to our senior service programs by completing a comprehensive intake. A part of the intake includes screening questions for our Department for the Aging case management program. Connections to services include access to counseling both in home and in the office, access to meals delivered to their home, home care, membership to our senior center, and services to families caring for an older adult, someone living with memory loss or grandparents caring for grandkids and more. The Intake Specialist will also have access to agency resources to properly refer a prospective client to the appropriate services. They will report to the Case Management Program Director and with day-to-day direction of the Care Connections Manager. Good record keeping, computer knowledge and time management skills are essential.
Responsibilities:
•        Conduct telephonic and in person intakes
•        Identify and assist clients with basic information on benefits and entitlements
•        Maintain complete and accurate data in Salesforce and Peer Place as required by DFTA and SCS
•        Make referrals to other available resources
•        Participate in trainings and conferences and agency meetings


Skills
Qualifications:
•        Experience in Human Service field
•        Exceptional communication skills with a strong focus on customer service
•        Must be bi-lingual in Spanish and English
•        Must have excellent interpersonal skills with an ability to establish rapport with staff, clients, informal supports and other community and government agencies
•        Must have minimum bachelor’s degree
