*$200.00 sign on bonus*

Job Summary: Full-Time Case Manager for Scatter Site Housing Program in the Bronx or Brooklyn Office. Will work directly with client caseload by providing services to severe mentally ill population in visits to client homes and in the community. Schedule Available: Monday-Friday. 9am to 5pm.

Essential Functions: Will coordinate care and maintain collateral contact with clients’ service providers, provide crisis intervention, and maintain case record around service provision. Must work well independently and take initiative in the execution of job responsibilities. This position requires travel throughout NYC using public transportation and operates within residential apartment buildings not owned or operated by the agency where staff enter clients’ residential units. The position uses standardized office equipment such as phones/cell phones, computers, fax machines, and copiers.

Minimum Education/Requirements: H.S. degree/GED and experience working with SMI population required.

Other Requirements:
1.        Undergo background clearances prior to employment including fingerprinting with New York State Division of Criminal Justice Services, State Central Registry (SCR) clearance, the Staff Exclusion List (SEL), and K-checks Exclusion Management.
2.        Ability to maintain clearances throughout the duration of employment.
3.        COVID-19 vaccination required.

Rate of Pay: $20.60/hour
