Job Summary: Full-Time Family Specialist to provide integrated care and transition assistance to individuals and family members on an interdisciplinary team serving 68 adults diagnosed with serious mental illness. Position involves telehealth sessions via office or remotely, as well as field work/home visits. This position operates 80 percent of the time in the community, rotating in an office setting as needed and for team meetings.
Essential Functions: Provides clinical support to consumers that is client-centered, recovery-oriented, trauma-informed, culturally competent, and sensitive to the experiences of those with behavioral health needs. Works with consumers and family members to provide psychoeducation and family needs assessments. Participates in daily team meetings to review client status and must complete all progress notes within 24 hours of service contacts (and complete other chart documentation as required). Complete required data reporting internally, as well as with external agencies such as Assisted Outpatient Treatment, Office of Mental Health and Department of Health and Mental Hygiene.
Minimum Education/Requirements: Bachelor level degree. Preferred candidate has a Master’s Degree in the Human Services field and is licensed or license-eligible. Must be detail and time oriented, have excellent communication skills and be able to document in an electronic health record.
1. Undergo background clearances prior to employment including fingerprinting with New York State Division of Criminal Justice Services, State Central Registry (SCR) clearance, the Staff Exclusion List (SEL), and K-checks Exclusion Management.
2. Ability to maintain clearances throughout the duration of employment.
3. COVID-19 vaccination required.
Salary Range: $45,000-53,000 annually (commensurate with experience)
