Job Summary: Full-Time Transition Specialist to provide transition assistance to individuals on an interdisciplinary team serving 68 adults diagnosed with serious mental illness. Position involves significant fieldwork and home visits. The Transition Specialist will assess individuals for transition readiness, provide linkage and escorts to appropriate outpatient providers as well as provide post discharge follow-up. Services are provided both individually and in group settings.

As part of the ACT you must be willing to go out into the community to provide services. This position requires clinical experience, and the desire to work as part of a team.
Minimum Education/Requirements: Bachelor’s Degree required, Master’s Degree preferred and have at least one year of experience in a comparable position. Must be detail oriented, have excellent communication skills and be able to document in an electronic health record.
1. Undergo background clearances prior to employment including fingerprinting with New York State Division of Criminal Justice Services, State Central Registry (SCR) clearance, the Staff Exclusion List (SEL), and K-checks Exclusion Management.
2. Ability to maintain clearances throughout the duration of employment.
3. COVID-19 vaccination required.
Salary Range: $44,000-47,000 annually (commensurate with experience)
