Job Summary: Full-Time Team Leader to provide integrated care to individuals on an interdisciplinary team serving 68 adults diagnosed with serious mental illness. Position is currently a hybrid model of remote work and office visits. Supervise and provide multi-disciplinary clinical and case management services delivered by a team of clinical and administrative staff with focus of assertively engaging hard-to-reach consumers with serious mental illness. Ensure that needs of consumers, ACT program goals, and contract objectives are effectively achieved. This role involves considerable professional initiative and judgment. Schedule Available: Monday-Friday, 9am to 5pm. Location Available: Brooklyn or Queens.
Essential Functions: Include but not limited to: adhering to ACT policies & procedures, agency policies and maintaining appropriate professional standards, managing day-to-day clinical operations and required productivity standards by implements procedures for monitoring of workflow and promoting enhanced staff performance, etc.
Minimum Education/Requirements: LMSW REQUIRED. At least two years of experience working in human services with individuals with serious mental illness and those with serious mental illness and co-occurring substance use disorders. Must be outcome oriented in a fast paced environment with deadlines. Able to work in the Microsoft Office Suite, should be proficient in Word, Outlook and Excel. Knowledge and experience of electronic health records (specifically AWARDS by Footholds) preferred.
Salary Range: $65,000 annually
