New Alternatives for Children, Inc. (NAC) is an award-winning not-for-profit agency in Midtown Manhattan, with a satellite Bronx office, dedicated to serving children and families with medical complexity, chronic health conditions, significant disabilities and behavioral challenges. To assist in achieving this mission, NAC is currently seeking an Assistant Director of Champions for Children’s Health (C4CH) to join its Champions for Children’s Health Department.
To assist the Senior Program Director in carrying out the standards set by CCF, New York State and City, along with NACs professional standards for the C4CH Program and to continue to create an atmosphere that reflects NAC’s mission.
Title of Position: Assistant Director of Champions for Children’s Health (C4CH)

Description of Responsibilities:
•Provide direct supervision to Care Management Supervisors.
•Be familiar with and provide necessary training on the C4CH Program’s mandates and regulations.
•Acclimate and support supervisors and care managers. Provide program leadership and be a steward of program and agency culture.
•Enroll new cases.
•Attend CCF meetings when Director is unavailable.
•Attend NAC Senior Leadership Extended Meetings as well as Critical Incident Review.
•Manage case load ratios and ensure billing requirements are met on a monthly basis. Maximize billing, problem solve and make case contacts when necessary.
•Assist with ensuring that all processes and functions of providing Care Management services (engagement, assessment, timely documentation and intensive care management) are being met, which may require attending Team Meetings, FTCs and/or Home Visits.
•Attend meetings at case planning foster care agencies and participate in home visits as necessary.
•Work with Quality Improvement to ensure that the C4CH Program meets standards and improves best practice.
•Assist with establishing and implementing policy and procedures to ensure efficient functioning and high quality service.
•Assist with interviewing and hiring.
•Responsible for any other duties reasonably required to meet the responsibilities of the position.
Required Education and Experience:
•Child Welfare experience in a supervisory position is preferred.
•Knowledgeable in work with medically fragile children, children with developmental/intellectual disabilities and/or serious emotional disturbance required. .
•Experience in program development a plus.
•Understanding of the C4CH Program
