Under the supervision of the Program Manager, the ROS is responsible for planning, coordinating, implementing and evaluating patient retention activities to reengage patients living with HIV/AIDS (PLWHA) within an HIV Medical Home. As a member of the Medical Care Coordination program team, the ROS collaborates to with MCC team members to identify patients who are eligible for MCC services. The ROS then contacts patients and provide brief, structured interventions to engage patients in care.
ESSENTIAL FUNCTIONS AND RESPONSIBILITIES: (This list may not include all the duties assigned.)
•        Works with MCC team identifying, locating, and reengaging patients back into HIV care.
•        Schedules follow-up appointments and conducts follow-up activities to ensure patients return for follow-up visits.
•        Assists in the coordination and delivery of patient-centered activities that focus on addressing health status, engagement and retention in care, adherence to HIV medications, and HIV risk reduction.
•        A Bachelor’s degree in public health, social work, psychology, counseling, nursing, human services, or health education OR A minimum of three years conducting outreach to vulnerable populations with the aim of improving health outcomes.
•        Experience and specific training/experience working with people living with HIV/AIDS preferred.
•        Proficient in MS Word and Excel software and tech savvy.
•        Bilingual in English and Spanish required.
•        Must be able to pass a background check to include a 7-year criminal, 10-year SSN & employer history reference check.
•        Excellent interpersonal skills.
•        Attention to detail.
•        Must be able to work flexible schedules.
•        Must have skills in research and development of operating policies and procedures.
•        Must take yearly flu shot and test for tuberculosis as required by the Centers for Disease Control and Prevention.
