ESSENTIAL DUTIES AND RESPONSIBILITIES
Build strong and genuine relationships with individuals receiving HCBS at Fountain House (“FH”).
Conduct HCBS intakes and assessments at the program site or in the community.
Devise Individualized Service Plans and Service Plan Reviews regularly, according to duration of services.
Communicate regularly with various Care Management Agency (CMA) staff to explain HCBS services provided at FH to ensure accurate HCBS designation and appropriate referrals.
Correspond regularly with various Managed Care Organizations upon submitting Prior Authorization and Concurrent Review Requests for service approval and delivery.
Set-up billing structures within EHR database.
Use EHR database to input demographics.
Document assessment services conducted in a timely fashion within EHR database.
Work one-on-one with individuals receiving HCBS, according to their service plan objectives at the program site and in the community.
Support other HCBS Staff to provide services effectively.
Maintain charts in an accurate and organized manner.
Meet strict deadline requirements.
Attend trainings, webinars and industry meetings to learn new guidelines; communicate and advocate for the evolving HCBS service provision.
Perform other related duties as assigned by supervisor

REQUIRED KNOWLEDGE, SKILLS, AND ABILITIES
Excellent verbal and written communication skills, including ability to effectively communicate with internal and external colleagues
Exemplary ability to build relationships quickly.
Excellent organizational skills.
Excellent computer proficiency (MS Office – Word, Excel, and Outlook)
Must be able to work under pressure and meet deadlines, while maintaining a positive attitude and providing exemplary customer service
Ability to work independently and to conduct assignments to completion within parameters of instructions given, prescribed routines, and standard accepted practices
REQUIRED AND PREFERRED EDUCATION, EXPERIENCE, AND CREDENTIALS
Bachelor’s degree required, preferably in health-related field.
Minimum of three (3) years of experience working with individuals with mental health conditions.
