Catholic Charities Community Outreach Services is seeking an experienced family coach/case manager to help clients move from crisis toward economic stability and improved wellness outcomes. Responsibilities and Duties:
-Provide crisis intervention to low-income New Yorkers around shelter, food, clothing, and other basic needs through the provision of financial assistance, goods, advocacy, benefits, resource access, information, and referral.
-Conduct assessments and create client driven service plans that address immediate need, intermediate and long-term goals.
-Develop a comprehensive knowledge of the complex social service system and coordinate care among a network of providers.
-Develop advanced knowledge about services to immigrants and seniors, homelessness, housing, healthcare, food, nutrition, etc.
-Enter information into data management systems.
-Attend and participate in monthly Department meetings, community meetings, trainings, and other agency-related activities.
-Bachelor's Required (with four years’ experience working in the social services field preferred) or entry level MSW.
-Spanish English or French/English fluency a plus
