We are seeking a compassionate, experienced Social Worker to join our Psychiatric Acute Care team at Scripps Mercy Hospital San Diego. Our diverse team is dedicated to serving the community with the highest quality of care.

Social Workers provide an important role within the hospital by assisting patients and families with personal and environmental problems which relate to mental illness or which interfere with their obtaining maximum benefits from available services.

The ideal candidate will have an LCSW and:

Enjoy working in a fast-paced 36 bed behavioral health adult inpatient unit.
Be knowledgeable about community resources
Enjoy facilitating patient groups and spending one on one time with patients
Provide/oversee psychosocial evaluation/consultation/referral and triage of assigned patients.

Conduct clinical assessments of patient/family/total situation to establish intervention strategies.

Develop resources such as educational programs for community agencies or hospital or ambulatory personnel.

Facilitate patient care conferences with other team members to review patient's goals of care, provide psychosocial support, assist with advance care planning and ensure proper referrals are made to community resources.

Ensure timely and effective transfer between facilities and nursing units; also facilitate transfer to appropriate level of care in order to ensure quality care, maximum reimbursement, and minimizing cost/day.

Conduct or attend discharge planning conferences with interdisciplinary healthcare team members.

Supervise other members of the social work team in providing the same level of discharge planning

Facilitate and direct peer group supervision with interdisciplinary clinical team.

Provide and/or coordinate in-service education and training for clinical staff.

Coordinate the education and training for social work interns.


This is a Full-Time, 8 hour day, varied schedule with flexibility to work an occasional weekend on case by case basis. This position is a combination of administrative duties and patient care. $3000 BONUS

Skills
Required:
LCSW
1-3 recent (within the last 3 years) of experience in an inpatient psychiatric hospital setting with a focus on discharge planning.
Knowledgeable and competent at applying mental health laws and regulations.
Content knowledge of LPS processes including involuntary detainment and the mental health conservatorship processes is required.

Preferred:
Post-MSW Palliative Care Certification and/or Oncology Social Work Certification.
