BronxWorks has an opening for a Hospitals Coordinator. Duties will center around liaising with Bronx hospitals and partner agencies that provide medical and psychiatric services in an effort to coordinate these services for street homeless clients. The services the program coordinator will be responsible for overseeing include, but are not limited to, medical care, psychiatric evaluation, psychiatric care, discharge planning, emergency shelter or stabilization bed referral, and medication management. The position will also be responsible for facilitating case conferences between BronxWorks, Bronx hospitals, and other homeless service providers.
The position requires a master’s degree in social work, public health, or related field. An LMSW is preferred. Experience in a residential setting where medical, psychiatric, and medication monitoring services are provided is preferred.
Provide direct services to clients, including intake, engagement, assessment, service plan development, referral, advocacy, counseling, exit summaries and follow-up
Provide written psycho-social summaries for clients
Document all interactions with, or on behalf of, clients
Maintain familiarity with program resources available on- and off-site
Provide group counseling as assigned by the program director
Coordinate client services with other social service, mental health, medical, employment, educational, child care and other providers
Conduct regular case conferences with clients and service providers as required
Complete program reports on a monthly basis, or more frequently as required
Report to, and meet with, supervisor on a monthly basis
Conduct home visits and other field work, as required
Perform additional duties as assigned
