Description
We have an exciting opportunity for a Clinic Services Support Staff member to assist in our CILA group homes! This position is responsible for ensuring the physical health and well-being of residential participants. Clinic Services Staff supports the QIDP and Director of Community Living Services as needed.
Responsibilities include but are not limited to:
•Ensure the accuracy of all medical need logs/tracking including physical exams, laboratory work, and treatments, as well as follow-up appointments are completed in a timely manner.
•Schedule and ensure all visits, Medical, Psychiatric, and Dental, are completed in a timely manner, properly tracked, and remain in compliance.
•Assist clients with Medical, Psychiatric, Dental visits to ensure all appointment follow up is communicated to the Home Manager.
•Assists with compliance of medication distribution and management in Residential program, and ensuring new medication prescriptions are issued to Residential program, and staff is aware of medications and instructed on dosage and compliance.
•Communicates to Agency nursing consultants in order to maintain compliance with staff medication training and files.
•Assures all documentation, resident and staff medication training files are filed correctly and in a timely fashion.
•Assists with podiatry clinic, senior eye clinic, and flu shot clinic.
•Being available and on call for emergency and after hour medical situations on rotating schedule with Clinic Support Staff as needed.
•Transports residential clients to offsite appointments/visits.
•Makes arrangements with other staff/Home Managers to handle appointments/visits when home coverage allows.
•Attends in-service trainings, team meetings, and other meetings.
•Completes participant attendance sheets and notes.
•Other Duties as assigned.


Skills
•College degree preferred.
•Minimum of 1 year working with the DD population.
•DSP certification is required.
•QIDP certification preferred, or be eligible.
•Available to be on call for emergency and after hour situations on a rotating schedule.
•Must have a valid IL driver’s license, ability to be insured under Oak-Leyden’s vehicle insurance
•Ability to pass all Background checks
•Excellent written and verbal communication skills
