Description
The Substance Use Disorder Special takes the lead responsibility for integrating dual-recovery treatment with the tasks of other team members.

DESCRIPTION OF DUTIES AND RESPONSIBILITIES:
Participates in the morning team meeting.
As part of the team, engages recipient in contacts at home or in the community.
Complete related substance use assessments and care coordination.
Provide individual counseling-recovery-wellness-education-reoccurrence prevention.
Document all interventions/ assessment timely and accurately.
Be available to off hours consultation for crisis intervention.

Daily:
Reviews all alerts in ECR daily.
Attends daily huddles   
Works toward key performance indicators
Documents all services provided in a timely manner

Weekly:
Attends Supervision

Monthly:
Ensures all reports are submitted accurately and timely all reports

KEY PERFORMANCE INDICATORS
Increase 340B referrals by 25% or 12 annually whichever is greater.
All program deliverables will sustain a minimum of 80%
Ensure all data collection are accurate and complete, and done in a timely manner

Skills
Proficiency in Microsoft Word, Bilingual preferred crisis; Flexible, Ability to meet deadlines; Use of Motivational Interviewing techniques/stage wise treatment and IDDT, WRAP, Knowledge of Harm Reduction, Ability to prioritize, follow policy and procedures Supports and promotes the ACT team mission, Understanding of OMH ACT regulations.
