-Contractor engages in weekly family and/or individual therapy with children and families per contracted hours arranged with Care Management Organizations
-Contractor completes all initial intake paperwork with families prior to engaging in services
-Contractor completes all progress notes within 48 hours of contracted service and submits them in the PerformCare system
-Contractor completes all treatment plans in the PerformCare system within one week of initial meetings with clients
-Contractor communicates regularly with Care Managers regarding updates, issues, or concerns related to clients
-Contractor attends all Child Family Team meetings as scheduling permits
-Contractor completes all encounter forms and submits them to Executive Director by 8:00pm on Wednesday in order to receive payment for services on time
-Contractor communicates any issues with families or care managers promptly with Executive Director
-Contractor completes WrapAround Training offered by Children’s System of Care if contractor chooses to work with clients in Middlesex County
-Contractor provides Executive Director with copies of updates insurance and licensure as needed.
-Contractor completes all initial paperwork including criminal background check, Medicaid policy, Driving Record check policy, Licensure check prior to beginning work with any clients
-Contractor will meet with clients in Hunterdon, Warren, Somerset, Middlesex, Mercer, Morris and Sussex (Depending on contractor availability and interest).
-Practical Insights, LLC currently works with care management organizations and mobile response units in the following counties: Hunterdon, Somerset, Warren, Middlesex, Mercer, Morris and Sussex.
Some clients may prefer to engage in tele-health sessions as opposed to in person sessions.
