Description
The Client Resources Coordinator III will assist CES -families into a “no-wrong-door network” by assessing the needs of families experiencing homelessness and linking them with the most appropriate housing and services to end their homelessness. The goal of the CES-Families Client Resources III is to streamline processes through which homeless families assess housing. The position also provides leadership to other Client Resources Coordinators, II and handles more difficult cases in serving clients through counseling, advocacy, and guidance. CRCIII staff is responsible for a triage screening, coordinating crisis housing, connecting families to supportive services to address any immediate needs such as emergency shelter, bridge housing and rapid rehousing services. Responsibilities: The following duties are also to be performed: •Assesses clients’ needs and helps them address and achieve their identified goals. •Develops problem-solving relationship with clients. Assesses and identifies needs and abilities of clients. •Completes assessments and makes referrals to mental health, BHS agencies within 72 hours for prescreening. •Develops Housing Stability plans and individualized Service Plan. •Works with other agencies and the CES-SPA8 to ensure clients receive all needed services. •Links clients to existing Agency and community resources, including Legal aid. •Must be able to perform Diversion strategies that prevents homelessness at the front door of the homelessness response system by helping people identify immediate alternative housing arrangements and, if necessary connecting them with services and financial assistance to help them return to permanent housing. Every effort should be made to divert clients to other housing solutions at their contact with the homelessness response system. •Records client-related information. •Compiles monthly statistics of services provided. •Inputes all clients and residents into Case Worthy, LB-HMIS and LA-HMIS. •Attends weekly and monthly case management meetings. •Monitors cases and plans for re-assessment biweekly. •Maintains confidentiality of client information and records.

Skills
•Master’s degree preferred, Bachelor’s degree required •Three to five years of related experience to include case management •Experience working with the homeless population. •Skilled in performing comprehensive case management •Ability to promote program services and network with partner agencies. •Must have a valid Driver’s License, Auto Insurance, and willingness to travel •Must be able to pass a Drug Screen & Department of Justice DOJ/FBI background check.
