Job Description
Provide on-the-job training and job coaching for individuals with disabilities in the community with the goal for individuals to become as independent as possible in their community jobs. Support individuals so that they can progress and learn how to perform their job accurately, efficiently, safely and successfully. This position may require significant travel to employment sites throughout Southern Los Angeles County
PRINCIPAL DUTIES:
In the community, assist and support individuals in the orientation to employer’s procedures, products, services, standards, policies, benefits, work schedule and opportunities.
Assist individuals in becoming as independent as possible in their community job. Assist individuals in building natural supports in their workplace. Train and teach clients how to advocate for themselves within a work environment.
Build and maintain strong working relationships with on-site leadership and staff at employer partners to provide a collaborative support model for individuals in programs and services. Needs to be aware of problems and risks and manage them appropriately on-site or with the assistance of program leadership.
Track key program metrics related to client productivity standards and expectations and be able to report on the data as needed.
Maintain effective communication and cooperative, professional working relationships with individuals, referral sources, families, caregivers, and Supported Employment Manager, phone calls, and in person.
Provide job-related counseling and guidance in career exploration, personal grooming, social skill awareness, effective money management, and general community skills.
Provide emotional and problem-solving support as needed; facilitate referrals to other services as needed. Monitor and support individual health and safety, communicating with caregivers and funding sources, and advocating when appropriate.
Provide job retention service, by targeting behavior or work conditions that are problematic. Develop plans to reduce problematic behavior or remedy situation and implement training strategies or adaptations to help individuals successfully retain their jobs.
Provide transportation assistance, including city bus and pedestrian safety training and coordination of scheduling on transportation services.
Develop, implement, and review Individual Plans that establish goals and training objectives.
Model or demonstrate lifting techniques safely and appropriately.
Complete additional responsibilities, as assigned.
PHYSICAL/SENSORY DEMANDS:
Full range of motion.
Ability to occasionally lift up to 50 pounds and model safe lifting techniques.
Repetitive use of hands and feet as needed for on the job coaching/training.
Vision standards required by Motor Vehicle Department to operate a vehicle.
