The main tasks of the Case Manager is to assist CalWORKs (CW) and other clients with referrals and linkages to necessities of life, to provide rehabilitative services, outreach, and advocacy. The Case Manager will evaluate services needed and refer clients to community resources. The Case Manager is in charge of ongoing agency paperwork, maintaining contact with designated resources such as educators, other mental health agencies and county entities.
·Experience in community mental health and Electronic Health Records (EHR) & Los Angeles County Department of Mental Health (LACDMH) systems/paperwork/policies.
·The Case Manager is also responsible for following up with clients to appropriate resource(s) and linkage in order to meet their mental health and psychosocial needs.
·The Case Manager is expected to participate in weekly team meetings and supervision and maintain the Agency’s required productivity expectations.
·The Case Manager is supervised by the CW Program Manager, who is under the supervision of the agency’s Program Director in order to ensure that quality and efficient mental health services are provided to clients.
