Haven Behavioral Hospital of Albuquerque is seeking a Social Worker or Licensed Therapist to join our team of devoted healthcare professionals.

They are able to see pass our patient's diagnosis and connect with them and see them for the person they truly are.

The Social worker, play a key role in ensuring our patients stay on the path of recovery. During the patient's stay the Social worker will assess and evaluate each patient, work with the providers to create individualized treatment plans, and ensure goals and objectives are strength based and consistent with the needs of the patient. They provide group and individual therapy sessions. Interact with the patients, their support system and treatment guardians.
When the patient is ready to return home, the Social Worker assumes an active role in discharge planning by identifying barriers to discharge. They Formulate discharge plans in coordination with the treatment team, patient, family, and outside agencies as appropriate. They establish and maintain cooperative relationships with community agencies and other resources.

• Two years of experience working with clinical multidisciplinary teams.
• Two years of Social work experience with at least one year experience working with behavioral health populations in an acute care setting.
• Clinical training in evidenced-based treatment modalities including individual, group and family interventions.
Master’s Degree in Behavioral Health Field
LCSW or LPC - preferred,
