Urban Pathways serves as the first point of contact for New York City's most vulnerable residents - people living on the streets, in subways, or in public areas. An innovative and data-driven leader, Urban Pathways is known for the effectiveness of our "housing first" approach. We engage individuals where they are in the cycle of homelessness.

Urban Pathways is seeking a part time CTI Licensed Social Worker for our Reentry Project. This innovative grant is designed to house ten seniors (age 50+) who have been incarcerated. This program, funded by a major foundation, is being launched as a pilot program and is intended to prove that supportive housing can prevent homelessness for those exiting incarceration. Urban Pathways intends to develop and document a replicable model for homelessness prevention that can be adopted by other organizations in the sector.

Reporting to the Project Coordinator, the CTI Licensed Social Worker's duties include:
•Utilizes Critical Time Intervention (CTI) intensive case management model to service seniors (aged 50+) with serious mental illness who are leaving or have recently left prison or jail and who are residing in shelter or are being discharged from a civil commitment to a psychiatric facility post-incarceration
•Assists in reviewing housing applications (HRA 2010E) as needed
•Conducts field work including but not limited to home visits and supportive housing sites
•Works closely with on-site case managers and other stakeholders (ACT/FACT/IMT teams, clinics, probation/parole, and/or other providers)
•Provides support and assistance to clients in connecting to health care, behavioral health and other community supports
•Develops and maintains a case load of up to 10 individuals in the first year
•Locates, develops and maintains relationships with community organizations and resources in collaboration with residential case managers
•Documents all client contacts following CTI model case notes
•Works with Project Coordinator to assess CTI model fidelity

•Licensed Master of Social Work
•Minimum two years of experience with homeless, supportive housing, reentry, alternative to Incarceration and/or social services fields
•Experience working with individuals experiencing homelessness, mental illness, substance abuse, and/or criminal justice involvement
•Training in Critical Time Intervention, Motivational Interviewing, and/or Cognitive Behavioral Therapy
•Experience with client data programs i.e. AWARDS preferred
•Flexible schedule including evenings, nights, weekends and holidays as necessary
•Valid Driver's License and clean driving record a plus
•Bi-lingual English/Spanish a plus
