Description
Isolation is used to separate people who have or are thought to have COVID-19 from those who don’t. Quarantine is used to keep asymptomatic persons who might have been exposed to COVID-19 away from others during the 14-day incubation period, while they watch for development of symptoms. Program participants are 18 and over adults who fall into one of four categories: 1. COVID positive by diagnostic laboratory test. 2. Person under investigation (PUI) with symptoms concerning for COVID-19 who either need to be tested or is awaiting test results. 3. Asymptomatic Close Contact (ACC) to a positive case who needs to quarantine for 14 days since last contact to the confirmed case. 4. Someone who is confirmed negative for COVID-19 but needs a place to isolate away from others they live with who are positive for COVID-19.
This is a 9-month position from October 2020 through June 30, 2021. This is a 24 hour/7 days-a-week program and we have morning, evening, overnight and weekend shifts to staff. Family Health Workers in COVID-19 Isolation and Quarantine (I&Q) Services provide direct care, engagement, support, and documentation for individuals and families in the I&Q program. As a part of a collaborative team, along with nurses and supervisors, safety navigators, and other Health Workers, this role will ensure participant safety and comfort while supporting them in adhering to the health and safety COVID-19 guidelines of the program. Family Health Workers will pay particular attention to the unique needs and challenges of families who are guests at the I&Q to ensure the safety and wellbeing of parents and children. The team works to engage participants in health resources, however great or small. Family Health Workers will model and demonstrate healthy COVID-19 transmission prevention behaviors, wearing appropriate PPE, and practicing physical distancing, while they observe and monitor participants and support them in meeting their needs in accordance with the program. Family Health Workers will be required to complete minimal documentation and assist in connecting our participants to ongoing services and supports, and others as they arise.
Treat program participants with respect and dignity, interacting with them in a caring, non-judgmental manner, utilizing harm reduction and trauma-informed care principles.
Conduct Family Needs Assessments to ensure the best effort are being made to accommodate individual family needs and ensure family safety.
Assess Family Safety, offer support, counseling, information and referrals to outside resources.
Engage participants in compassionate ways to build motivation toward health initiatives in their own lives.
Respond to participants’ needs, ensure that COVID-19 precaution protocols are being addressed, and, to respond to crises (e.g. conflict) de-escalation interventions.
Attend to I&Q program operations to help maintain safe space for participants.
Provide COVID-19 education and support, as well as provide harm reduction focused health engagement, health improvement activities, disease prevention, and other areas of participant health.
